Make a Referral

Every person who is referred to our service will be involved in the assessment process. The Service User and their family will be fully involved and supported throughout the referral process.

Each referred person will have a comprehensive multi-disciplinary assessment prior to admission; this will determine suitability for the service and determine the care package required to meet their needs.

We take referrals from all over the country and regardless of race, religion or ethnicity.

Our referral process follows a step by step approach;
1 - Use the online form here or contact Gail Brunskill on 0161 862 9521
2 - You will be contacted with 2 working days (Excluding bank holidays)
3 - A pre- admission assessment will be carried out within 5 working days
4 - A placement decision will be made within 4 days post completed assessment
